Eligibility
Eligibility for the state Medi-Cal Program is determined through the Department of Health & Human Services Social Services. Medi-Cal benefits are administered locally through Partnership HealthPlan of California. Medi-Cal serves low-income individuals and families.
Application
Applications are accepted in multiple ways.
Click, call or come in to apply! Visit BenefitsCal.com and coveredca.com, call the Eureka Call Center at 1-877-410-8809 or come into one of our office locations. You can also mail in your application to 929 Koster St. in Eureka or fax a completed application to 707-269-3598.

BenefitsCal.com is a new way to apply for, view and renew benefits for health coverage, food and cash assistance.
You may choose to have a face-to-face interview, telephone interview, or you can complete the entire process by mail. You can request an application by calling 1-877-410-8809 at any time.
Medi-Cal applications are also accepted at all hospitals and numerous clinics throughout the county.
