Order Vital Records Online

For your convenience, Humboldt County has partnered with an independent company, VitalChek Network, Inc. through which an online vital record order may be processed.

• All major credit cards are accepted, including American Express®, Discover®, MasterCard® and Visa®
• VitalChek charges an additional $7.00 for their service and $20.00 for express UPS overnight.  If you do not require overnight service, you must uncheck the corresponding box. Please note that delivery options cannot be changed once your order is submitted.
• All applicants requesting authorized copies must submit a notarized Certificate of Identity before the order will be processed.

IMPORTANT: VitalChek users please be advised! Your certificate order WILL NOT BE PROCESSED without the return of the "sworn statement" sent to you as an attachment with your order confirmation email.  You may fax or email the completed sworn statement back to the Clerk Recorder’s office to avoid delays.
